This questionnaire is to apply to the Confirmation programme 2022.
This invitation is open to candidates in Year 10 and above for the Sacrament of Confirmation.
Information provided in this Questionnaire, together with all other personal data held about the individuals by the Parish and the Diocese of Arundel and Brighton, is processed in accordance with the Diocese’s Privacy Notice, which is available at www.dabnet.org.
Registration Process
The registration process will be completed in two stages:
Stage 1:
Completion of Questionnaire.
Stage 2:
Submit copy of Baptism certificate via email to:
Confirmation@rcdab.onmicrosoft.com
After this 2 stage process is complete, you will be invited to meet with one of the Priests.
A donation to the Confirmation programme towards the cost of documentation and administration can be made at this meeting.
Before the 'Confirmation candidate' comes to see the Priest, he or she should be able to say the:

Additional Information:
The information in this questionnaire will be used to facilitate and deliver the Confirmation Programme and to update the Parish Database.
Group photographs may be taken during the course and / or event in line with the Church's policy. If you do not consent to this please let the course leaders know.
